I need reliable transportation to make doctors appointments to regain my health!

My goal is to regain my health. I need a $5 ride free card to travel to doctors appointments.


I am a senior citizen, 65 years of age, and also disabled; I have been physically disabled since 1989. I am currently homeless and have been since March 13, 2012.

My goals are to find housing and regain my health that being homeless has taken away.

I need one ride free card to travel. I have many doctors appointments and need a reliable way of transportation to make the appointments.

Meeting this need will help me because I will be able to attend doctors appointments and travel to food pantries to receive food. By attending appointments, I hope to better my health and have a healthy future.

I have known Carrole through the case management program at Chicago Lights Elam Davies Social Service Center. She recently started working with us on bettering herself and her health. She has a very upbeat attitude and is willing to work hard to better herself. Transportation is a necessity to her success and it would be wonderful to see her receive this help.
