Overview

I need a copy of my birth certificate. This includes the fee for the replacement birth certificate from the State of Illinois.

My goal is to establish permanent housing -- the only obstacle I am currently facing is obtaining a birth certificate. Meeting this need will allow me to obtain my apartment on December 3 and will keep me off the cold, Chicago streets.

My current situation is that I am currently homeless, but moving into an apartment on December 3. I am in need of a birth certificate as a document to secure my senior housing.

I'm from Chicago.

I'm most proud of my four children: two have graduated from college, one from Northwestern and one from Bradley. Something interesting about me is that I was a two-time city champion in speed skating during high school.

I began my internship at Chicago Lights Elam Davies Social Service Center in mid-August and I met William shortly after. Meeting this need is important because once William has a copy of his birth certificate, he is able to secure his housing. William has already found a beautiful apartment and he is so excited to move in. William is one the happiest people I have ever met and he deserves this need to be met.
