Overview
I'm a 38-year-old homeless man, living on the streets of Chicago. I've been homeless on and off for 10 years. I've been actively working with a Case Manager while also looking for employment and housing.
My goals are to secure employment and to find permanent housing.
I need funding to get my birth certificate and criminal history report so I can be prepared when applying for employment and housing opportunities.
Meeting this need will help me because these documents are vital to the application process. Once employed, I'll be able to finally find housing and get off the streets.

I have known Shawn through the Sunday Night Supper Program, a community meal that I coordinate. Recently he expressed interest in Case Management services so he can find stable housing and employment. Shawn is incredibly motivated and is ready to make changes in his life. Getting these documents is the first step in making these changes and reaching his goals.
