I am a Social Security Income recipient. I have several health issues which requires me to have the assistance of a home health aid seven days per week for four hours per day. Due to my health, I am currently unable to maintain employment.

My goals are to keep all medical appointments as scheduled and attempt to maintain a healthy lifestyle.

I need to obtain a medical identification bracelet. Because of my poor health and needing to travel to different appointments by myself, it is necessary for me to identify my ailments in case of an emergency.

Meeting this need will help me be able to receive the care I need.

I recently started working with Robert who disclosed that he has several chronic health issues which would require immediate medical assistance should he have a crisis and is without his home health aide. In addition to his medical issues he is also dealing with his father whom he revealed is currently battling cancer, has blood clots and cannot take care of himself thus relying on Robert for assistance. Having said bracelet could potentially save Robert's life.
