Overview
I am a 52 year old maintenance worker on the weekends. I am from Chicago and have lived here for many years. Because of my limited income, my mother is allowing me to stay with her, but I would really like to be self-sufficient.
My goals are to have my own place to live and have a permanent job. I would like to be self-sufficient and independent.
I need to get funding to obtain a state identification card.
Meeting this need will help me because without an ID I am unable to apply for a job and housing program.

I know Chris from the short term case management program at the Elam Davies Social Service Center. Chris has a number of goals, but he really needs and ID to achieve his goals. As I work with Chris, we are hoping to get him into his own housing, but obtaining an identification card is the first step toward accomplishing his goals. Chris is very motivated toward achieving his goals and is working hard to become more independent and self-sufficient.
