Overview
I am a 48-year-old Chicago resident. I am currently homeless and experiencing lung cancer. My illness requires that I receive treatment, however it is difficult without income and transportation to get to my various appointments.
My goals are to make all of my medical appointments, take care of myself to the best of my ability, and eventually get back on my feet.
I need a 30-day fare card in order to get to my medical appointments. With the month of free transportation, I hope to make enough money to pay for the next month's fare card.
Meeting this need will help me because it will provide me with the opportunity to receive the health services that I need currently.

I have gotten to know Brian through our mobile street outreach program for the last few months. He is currently suffering from lung cancer and is having a lot of trouble making it to his appointments. Brian deserves the same access to health care as everyone else and he would greatly benefit from being able to get around the city a little easier.
